Chronic Disease GP Management Plans (GPMP)
To be eligible for this service patient must have a chronic or terminal condition. Common conditions include:
- Asthma
- Cancer
- Cardiovascular disease
- Diabetes
- Kidney disease
- Musculoskeletal conditions
- Stroke
This list is not exhaustive, diseases that are present for or will be greater 6 months are eligible. It is important to discuss with your doctor to see if this is appropriate for you.
A GPMP encourages patients to take responsibility for their own care, also allowing them to be able to identify things they can do to achieve their goals.
Those who are eligible for this service have 3 monthly reviews with their doctor and have an annual review which includes a nurse to conduct preventative testing such as ECG (test that shows how your heart is beating), ABI (checks blood flow in legs), and Spirometry (lung function test) for 20-30 minutes and review with doctor for 15 minutes completing the management plan.
Information that is discussed in a GPMP are your specific health care needs, current health problems and the relevant goals of care and further treatment required.
As part of this service patients may benefit from referrals to allied health services such as physiotherapy, podiatry, diabetic education. Patients can receive 5 individual services each calendar year. These appointments enable you to receive reduced cost care and improve access to allied health. Please work with your doctor to ensure you receive the appropriate care for your condition(s).
These services are bulk billed and significantly improve low-cost access to a regular doctor for your care. Having 1-2 doctors that know you well, will significantly increase your standard of care and future health.
