  • 75-year-old health assessment is an in-depth assessment of the patient, providing a structure way to assist in identifying health concerns and conditions that may be potentially preventable to improve your health and quality of life. This assessment identifies a broad range of factors that influences a person physical, social, and psychological function. 
  • This assessment helps to identify risk factors and where the patient may benefit from further health management such as community services, social isolation, and nutrition. 
  • This assessment includes specific components of the patient health such as current medication management, continence, immunization status, assessing patients’ physical functions, cancer screening, Activities of Daily Living, falls assessment, psychological function such as patients’ mood and memory screening. Physical assessments such as height, weight, and blood pressure.
  • As part of this assessment, you will be allocated 40 minutes with a nurse to complete preventative assessments such an ECG, ABPI, and spirometry. You will then be allocated a 30-minute appointment with a doctor to complete assessment and have in-depth discussion about our health and wellbeing. 
  • This Assessment is different to a GPMP as it covers a multitude of other health issues that require more in-depth discussions. Extremely important aspects include memory testing, advance care planning, cancer screening, medication reviews and consideration of home supports.
